Please prepare : last name and first names of your child, your child's 13-digit Luxembourg national identification number (matricule*), email, mobile phone, postal address, name and code of the general practitioner who issued the prescription.
(*) If you do not have a matricule, you will not be able to make an appointment online. In this case, please contact the Ligue by e-mail tracing@ligue.lu or by phone at +352 22 00 99-66 (open from 8.00 to 12.00 and from 13.00 to 17.00).
